Assessment and Management of Patients With Biliary Disorders

1. Evelyn Bean, 52 years of age, is admitted to the same-day surgery unit for an elective laparoscopic cholecystectomy. The patient presents with jaundice of the skin and sclera. The patient’s urine is dark and the patient stated that she has clay-colored stools. She stated she has occasional colicky pain in her right upper quadrant of her abdomen radiating to her back. The patient had preadmission testing 1 week ago and the results are on the patient’s chart. The record of patient education and an informed written consent are also on the chart. (Learning Objectives 2 and 3)
What additional procedure will be performed if the common bile duct is obstructed by a gallstone?
The nurse in the same day surgery unit provides the discharge instructions to the patient and family before the patient goes for surgery so the patient is fully awake to receive the instructions and ask questions. What written and verbal instructions should the nurse provide?
2. Joe Harrison, a 62-year-old bank executive, presents to the emergency room with severe abdominal pain. He describes the pain as excruciating, and indicates it is located in the mid epigastrium with radiation into his back. The patient states he has not eaten anything in the past 24 hours, but 2 days ago attended a wedding dinner and consumed a large meal and about 4 to 5 alcoholic beverages. Mr. Harrison admits to being a “social drinker,” ingesting 2 to 3 alcoholic beverages several days a week. Based on his clinical presentation and history, Mr. Harrison is admitted with a diagnosis of rule-out pancreatitis. (Learning Objectives 4 and 5)
Blood and urine samples are sent to the lab, and the results indicate that the serum amylase and lipase are markedly elevated. How do these findings correlate to the diagnosis of acute pancreatitis?
The nurse notes that Mr. Harrison’s stool is pale and bulky, while his urine is a dark tea color. Correlate these findings to the pathophysiology of acute pancreatitis.
Mr. Harrison’s WBC is 18,500, and his serum glucose is 325. His LDH is 300 IU/L and the AST is 120 U/mL. Based on the Criteria for Predicting the Severity of Pancreatitis, what is Mr. Harrison’s mortality risk?
Mr. Harrison develops hypocalcemia secondary to the acute pancreatitis. What nursing interventions should the nurse implement related to this complication?

business Hw

You are to research and write a short report on two current business articles (so it will be 2 short reports each of which is no longer than two pages) regarding a contemporary event or events (or coverage of an industry or individual) each of which highlight a course concept or concepts addressed this semester. (Examples: Design Thinking, The Entrepreneurial Mindset, Failure, etc.) The articles must be from a business publication such as the Wall Street Journal, Inc., Forbes, FastCompany, Entrepreneur, or other national business publication. The article should not be dated any earlier than January 2020 (so it should be relatively current). The article should be a feature article (delves a little deeper into the topic) and not a simple news story. Your one-page write up must contain a one paragraph summary of the article and answer the following questions:
1. What is the course concept that the article illustrates?
2. How does the topic of the article relate to that course concept?
3. What did you learn from the article about the course concept that you might not have known before or how did the article reinforce the course concept?
4. Any other takeaways you gained from the article
You need to properly cite the articles that you have covered in your paper and any other sources that you use including the class textbook that you may need to reference to describe the course concept. You need to do your best to use proper grammar and not to use the first person. (Don’t say “I” or “you” or “we” – you’d write “one” instead) in your report.

Management of Patients With Oral and Esophageal Disorders

Management of Patients With Oral and Esophageal Disorders
1. George Smith, a 55-year-old patient is admitted to the intensive care unit after a thoracotomy approach was used for an esophagectomy to remove an early stage adenocarcinoma of the distal esophagus and gastroesophageal junction. The patient has a history of GERD and Barrett’s esophagus. The patient sought medical treatment for dysphagia with solid foods, feeling that there was a lump in his throat and substernal pain with swallowing and subsequent regurgitation of undigested food and the development of hiccups. The patient has no other medical problems. (Learning Objectives 6 and 7)
Describe the esophageal cancer that the patient has and how it relates to his history of GERD and Barrett’s esophagus.
Explain the rationale for the early symptoms and late symptoms of esophageal cancer.
What course of cancer treatment should the nurse anticipate that the patient in this case study would have?
What nursing care should be provided for the patient in the intensive care unit in the early postoperative period?
Explain the care the nurse should provide when the patient begins to eat.
2. Mr. Owens is a 62-year-old man who underwent a neck dissection yesterday due to cancer of the mouth. You are the nurse assigned to care for Mr. Owens during his first postoperative day. Initial assessment finds Mr. Owens sitting up in bed; he is drowsy, but oriented ×3 when aroused. He has significant edema in his neck and mouth area, but does not appear to be in respiratory distress. His respiratory rate is 16 to 18, and his oxygen saturation is 96% on 40% oxygen via face tent. He has two peripheral IV lines both infusing Lactated Ringer’s solution at 75 mL/hr. Two Jackson Pratt drainage tubes are partially filled with serosanguinous drainage. (Learning Objective 5)
What is the rationale for the patient being placed in Fowler’s position after surgery?
The nurse notes that there has been 240 mL output in the drainage tubes during the first 24 hours after the surgical procedure. What should the nurse do?
Postoperatively, the nurse identifies that the patient is at risk for imbalanced nutrition, less than body requirements related to anorexia and dysphagia. The nurse instructs Mr. Owens to eat soft food and suggests that he tilt his head to the unaffected side to facilitate swallowing. What is the rationale for these instructions?
In reviewing Mr. Owens’ medical record, what findings in his health history are associated with development of oral cancer?

Question for statistics

The study collected current and beginning salary data for each of the 474 employees and the researcher conducted a statistic and the results are presented below. Using the five steps of hypothesis testing, explain what the researcher might have done, including the appropriate analysis, and interpret the results. Are there any problems with this study? If so, explain what they are. Beginning salary mean = $17,016.09, SD = $7,870 Current salary mean = $34,419.57 SD = $17,075 Test statistic = 35.036, p <.05

What nursing assessment should the nurse provide after the cast has been applied?
What nursing interventions should the nurse provide?
2. Sue Newhart, a 55-year-old patient, is admitted to the medical-surgical unit after a total hip arthroplasty due to osteoarthritis. (Learning Objective 7)
What considerations should the nurse follow when positioning the patient in bed after hip surgery?
What nursing interventions should the nurse provide the patient?
